You must be an active associate and enrolled in a Sysco-sponsored medical plan in 2018 and 2019 to be eligible to receive your 2019 Wellness Reward.

If you are eligible and you cover your spouse under your Sysco-sponsored medical plan in 2018 and 2019, then your spouse is eligible to participate and earn a reward in the amount of $200.

If your spouse is also a Sysco associate and he or she is covered under your Sysco-sponsored medical plan in 2018 and 2019 (associate & spouse coverage), then your spouse is only eligible for the spouse Wellness Reward in the amount of $200. Each individual may only earn a single Wellness Reward.

If your spouse is also a Sysco associate and he or she enrolls in a separate Sysco-sponsored medical plan in 2018 and 2019 (not associate & spouse coverage under your Sysco medical plan), then your spouse is eligible for the associate Wellness Reward in the amount of $400. Each individual may only earn a single Wellness Reward.

Yes. You and your spouse must be enrolled in a Sysco-sponsored medical plan in 2018 and 2019 to be eligible to receive your 2019 Wellness Reward.

You must be enrolled in a Sysco-sponsored medical plan in 2018 and  2019 to receive your 2019 Wellness Reward. If you actively waive your Sysco medical plan coverage for 2019 during the 2019 benefits enrollment period, you will not receive the Wellness Reward in 2019, even if you have completed the requirements to earn your 2019 reward.

Regardless of how you choose to receive your reward — as a credit to lower the medical plan premium taken from your paycheck or as a deposit in your healthcare expense account— you must be enrolled in a Sysco-sponsored medical plan in 2019 with a medical plan premium and a healthcare expense account to receive your reward.

If you complete a biometric screening or submit your “I Got My Physical” Form by the October 1, 2018 deadline you will receive your reward as a credit to lower the medical plan premium taken from your paycheck in 2019. If you would rather receive your reward as a deposit in your healthcare expense account, you can make this election in the Total Rewards Café during the annual benefits enrollment period in 2018.

The total amount of your Wellness Reward (earned by you and your spouse) will be divided by the number of paychecks you receive in 2019, and your medical plan premium on each paycheck will be reduced by this amount. For example if your total Wellness Reward amount is $600 and you receive 26 paychecks in 2019, your medical plan premium would be reduced by $23.08 on each paycheck in 2019 ($600 divided by 26 equals $23.08).

Depending on which Sysco-sponsored medical plan you are enrolled in, you have one of two types of health expense account—a Health Savings Account or a Health Reimbursement Account. You can use the money in your account to pay for out-of-pocket medical expenses. Both of these types of accounts allow you to roll your unused money over from year to year without losing it.

The 2018 Wellness Rewards Program window ended on November 1, 2017. If you and your spouse completed the requirements by November 1, 2017, and earned your Wellness Reward for 2018, your reward will be deposited in your healthcare expense account in January 2018! Don’t be concerned if your Wellness Reward amount is not displayed in the enrollment system on the Total Rewards Café in 2017. Your 2018 Wellness Reward amount will not appear in the system until January 2018.

Please note: If you and your spouse completed the requirements early — by October 1, 2017 — and did not choose to receive your reward a different way during 2018 Benefits Enrollment, you will receive your reward as a credit to the medical plan premium taken from your paycheck each pay period in 2018.
