If you are eligible and you cover your spouse under your Sysco-sponsored medical plan in 2019 and 2020, then your spouse is eligible to participate and earn a reward in the amount of $200.

If your spouse is also a Sysco associate and he or she is covered under your Sysco-sponsored medical plan in 2019 and 2020 (associate & spouse coverage), then your spouse is only eligible for the spouse Wellness Reward in the amount of $200. Each individual may only earn a single Wellness Reward.

If your spouse is also a Sysco associate and he or she enrolls in a separate Sysco-sponsored medical plan in 2019 and 2020 (not associate & spouse coverage under your Sysco medical plan), then your spouse is eligible for the associate Wellness Reward in the amount of $400. Each individual may only earn a single Wellness Reward.

You must be enrolled in a Sysco-sponsored medical plan in 2019 and  2020 to receive your 2020 Wellness Reward. If you actively waive your Sysco medical plan coverage for 2020 during the 2020 benefits enrollment period, you will not receive the Wellness Reward in 2020, even if you have completed the requirements to earn your 2020 reward.

Regardless of how you choose to receive your reward — as a credit to lower the medical plan premium taken from your paycheck or as a deposit in your healthcare expense account— you must be enrolled in a Sysco-sponsored medical plan in 2020 with a medical plan premium and a healthcare expense account to receive your reward.

You can earn your 2020 Wellness Reward one of two ways:

  • Get an annual physical at your primary care doctor’s office. Then, let us know that you got your physical by completing the “I Got My Physical” Form─OR─
  • Get a biometric screening. To complete your screening at Quest Diagnostics, you are required to schedule an appointment online and enter registration key SYSCO. If you schedule your appointment online using the Sysco registration code, you will not need to fill out a form. Your wellness activity will be automatically submitted. Please note, if your doctor sends you for blood work at a Quest Diagnostics, but you do not schedule an appointment online and use the SYSCO registration key, you must complete the “I Got My Physical” Form. If you complete your screening at a non-Quest Diagnostics location, you must submit the “I Got My Screening at a Non-Quest Location” Form.

It will give you a clear picture of your current health status and a roadmap to help you achieve your health goals. We feel it is so important that we are adding it as an option to fulfill the Wellness Reward requirements that reward you and your covered spouse for your efforts to maintain your health!

If your spouse is covered under your Sysco-sponsored medical plan in 2019 and 2020, and he or she completes the reward requirements by October 1, 2019, you will receive a $200 reward in 2020. However, you will not receive the $400 associate Wellness Reward, unless you also meet the requirements. Don’t miss out on the additional $400!

No. Although everyone should get a physical as part of staying healthy, children are not required to meet the requirements in order for you to earn your 2020 Wellness Reward and children are not eligible to earn a separate award.

If your spouse is covered under your Sysco-sponsored medical plan in 2019 and 2020, he or she may also get an annual physical with a primary care doctor (and submit the "I Got My Physical" Form) or a biometric screening at Quest Diagnostics by October 1, 2019, to earn a Wellness Reward in 2020.

Remember, to complete your biometric screening at Quest Diagnostics, you are required to schedule an appointment online and enter registration key SYSCO. If you schedule your appointment online using the Sysco registration code, you will not need to fill out a form. Your wellness activity will be automatically submitted. Please note, if your doctor sends you for blood work at a Quest Diagnostics, but you do not schedule an appointment online and use the SYSCO registration key, you must complete the "I Got My Physical" Form.

Yes. You can see your primary care doctor for an annual physical or, if a woman, your annual well-woman exam counts too! Simply let us know that you had your visit by completing the “I Got My Physical” Form.

No. Your DOT physical is for job-related purposes only and does not qualify you to receive a 2020 Wellness Reward. You must get an annual physical with your doctor or a biometric screening through Quest Diagnostics to receive your 2020 Wellness Reward.

No. If you have not already received a screening or physical in 2019, you must get an annual physical with your doctor or a biometric screening through Quest Diagnostics to receive your 2020 Wellness Reward.

If you are covered under a Sysco-sponsored medical plan in 2019, Sysco pays 100% of the cost of in-network preventive care for you and your covered spouse. So, whether you schedule your biometric screening through Quest Diagnostics or receive an annual physical at your in-network primary care doctor or OBGYN, these services are covered at 100% through your Sysco-sponsored medical plan. Print the “Letter to your Doctor” and give it to your doctor when you receive your annual physical to ensure that your doctor codes your visit correctly (as preventive).

To ensure that your doctor codes your annual physical as preventive, print the “Letter to your Doctor” and present it to your doctor on the day of your appointment. Also, when scheduling your annual physical, make sure you tell the doctor’s office that you are coming in for your preventive annual physical (age/gender appropriate preventive care screening).

If you get an annual physical or a biometric screening and submit your “I Got My Physical” Form (if applicable), you will earn $400. If your covered spouse completes either of the required actions too; you will earn an extra $200. That’s a total of $600.

The total amount of your Wellness Reward (earned by you and your spouse) will be divided by the number of paychecks you receive in 2020, and your medical plan premium on each paycheck will be reduced by this amount. For example, if your total Wellness Reward amount is $600 and you receive 26 paychecks in 2020, your medical plan premium would be reduced by $23.08 on each paycheck in 2019 ($600 divided by 26 equals $23.08).

Yes. Your 2020 Wellness Reward is determined solely by your completion of an annual physical with a primary care doctor or a biometric screening. You are not required to achieve healthy ranges or participate in health management programs to receive your reward. However, we do encourage you to review your screening results with your primary care doctor and discuss ways to help you improve or maintain your health.

If you receive an annual physical at your doctor’s office, make sure you submit the “I Got My Physical” Form so we register that you received your physical. To complete your biometric screening at Quest Diagnostics, you are required to schedule an appointment online and enter registration key SYSCO. Now, go the extra step, even though it’s not required: take your biometric screening results to your primary care doctor and discuss steps that you can take to improve or maintain your health. Remember, you and your spouse must be enrolled in a Sysco-sponsored medical plan in2019 and 2020 to be eligible to earn your 2020 Wellness Reward.

If your doctor orders lab work at a blood lab other than Quest Diagnostics in 2019, you are still eligible to earn your 2020 Wellness Reward. Simply submit** the “I Got My Screening at a Non-Quest Location” Form before October 1, 2019, to report your screening.

If you’ve completed your biometric screening, seeing a doctor is not a requirement to receive your 2020 Wellness Reward, but we encourage you to take this additional step. Why? Because we care about you—and you should, too. We know many of you have physical jobs and we want you to have a partner in managing your health. Even if you are young, there are benefits to establishing a relationship with a primary doctor and creating a baseline of knowledge about your current health status.

No. Your health information is private and protected under federal law. The Sysco Plan will only know that your biometric screening took place. Any information discussed will remain between you and your doctor.

If you got your biometric screening at a Quest Diagnostics location, log on to  my.questforhealth.com.

If you got your biometric screening at a non-Quest Diagnostics location, contact your screening provider to receive a copy of your results.

If you participated in a Sysco-sponsored onsite screening or used the SYSCO code to register at a Quest Diagnostics center, Quest Diagnostics will mail you a “My Guide To Health” report that contains your biometric screening results within 7-10 business days after you complete your screening. If you do not receive a “My Guide To Health” report mailed to your home in this timeframe, you may contact Quest Diagnostics to be certain that they have a record of your biometric screening. If Quest has your screening on file, then the Sysco Plan has been given notice that the screening requirement has been satisfied.

The 2018 Wellness Rewards Program window ended on November 1, 2017. If you and your spouse completed the requirements by November 1, 2017, and earned your Wellness Reward for 2018, your reward will be deposited in your healthcare expense account in January 2018! Don’t be concerned if your Wellness Reward amount is not displayed in the enrollment system on the Total Rewards Café in 2017. Your 2018 Wellness Reward amount will not appear in the system until January 2018.

Please note: If you and your spouse completed the requirements early — by October 1, 2017 — and did not choose to receive your reward a different way during 2018 Benefits Enrollment, you will receive your reward as a credit to the medical plan premium taken from your paycheck each pay period in 2018.
