You must be enrolled in a Sysco-sponsored national medical plan, administered by BCBS, to access these programs.
You have two dental plan options through MetLife: Basic and Premium. The main difference between the two plans are the deductibles, out-of-pocket maximums and orthodontia coverage. To learn about the exact differences, click here.
Seeing a dentist in the MetLife network will get you the most bang for your buck. When you make an appointment, share that you have MetLife coverage and provide your name and date of birth, along with MetLife’s phone number so they can verify your eligibility: 1-800-942-0854. Keep in mind, there is no ID card for the dental plan. Find an in-network provider in the Total Rewards Café or on metlife.com/mybenefits.
For dental services that cost more than $300, contact MetLife to receive a pre-determination of benefits. Then you’ll know what you can expect to pay out of your own pocket.
