Basic Life and AD&D Coverage

Sysco provides Basic Life and Accidental Death & Dismemberment (AD&D) Insurance, through MetLife, to protect your family in the event of your death or disability.

Sysco pays the full cost of your Basic Life and Basic AD&D coverage. These benefits are automatically yours as an eligible Sysco colleague.

  • Basic Life Insurance provides coverage equal to 1.5x your eligible earnings (rounded up to the nearest $1,000), up to a maximum of $225,000.
  • Basic Accidental Death & Dismemberment (AD&D) Insurance provides coverage equal to your eligible earnings (rounded up to the nearest $1,000), up to a maximum of $150,000, if you lose your life or suffer dismemberment as a result of an accident.

Supplemental Life and AD&D Coverage

You can purchase additional Life and Accidental Death & Dismemberment (AD&D) Insurance for yourself, your spouse/domestic partner, and/or your child(ren).

Costs for Supplemental Life and Voluntary AD&D Insurance are based on the colleague‘s age as of January 1 each year. Coverage costs can be found by logging in to the online Sysco Benefits Center.

The value of coverage that exceeds $50,000 will be included in your gross income, subject to Social Security and Medicare taxes.

Beneficiaries

A beneficiary is the person you choose to receive your Life and AD&D Insurance benefits in the event of your death. Designate a beneficiary in the online Sysco Benefits Center. You need to select a beneficiary regardless of whether you‘ve completed a paper form in the past.

Filing Claims

You can find instructions for filing claims here. You can submit documents for a claim by calling the Sysco Benefits Center at 1-800-55-SYSCO (1-800-557-9726), available 7 a.m. – 7 p.m. CT. Select option 8 for the Survivor Support Team.

Below is an overview of Supplemental Life Insurance benefits for you, your spouse/domestic partner, and children. The guaranteed issue amount is the coverage you can elect without providing Evidence of Insurability.

Voluntary AD&D Coverage

  • Spouse/domestic partner coverage: 50% of your Voluntary AD&D coverage if you have dependent children, or 60% if you don’t.
  • Child coverage: 15% of your Voluntary AD&D coverage if you are married, or 20% if you are unmarried. The maximum amount for any child is $150,000.
